PHILHEALTH CIRCULAR
No OA-i>nTO:ALL EMPLOYERS IN THE GOVERNMENT AND PRIVATE
SECTORS, AND ALL OTHERS CONCERNED
SUBJECT :Conduct Of "Adopt EPRS And Win-A-Prize!" Promo For The
Employed Segment
1.RATIONALE
1.1.Section 6(f) of the Implementing Rules and Regulations (IRR) of the National hlealth Insurance ^Act
of 1995 (Republic Act No. 7875 as amended by Republic Act No. 9241) states that
PhilHealth shall " [e'jstablisb and maintain an updated membership and contribution database..!' Thus,
PhilHealth Circular No. 25, series of 2012 has been issued, mandating the employers to
mnndatorily adopt the Electronic Premium Reporting System (EPRS) in the preparation and
submission of Employer's Remittance Report (RF-1), and phasing out of other reporting
scheme.
1.2.This Circular is being issued to provide additional incentives to employers to comply with
the EPRS registration requirement and actively adopt the EPRS as their primary tool in
reporting their premium contribution remittances to PhilHealth.
2.PROMOTIONAL GUIDELINES
2.1.Promotion Period
2.1.1. The promotion period shall be from September, 2012, to December 31, 2012.
2.2.Eligibility
2.2.1. Qualified to join are government agencies and private business entities diat:
2.2.1.1.Have, been enrolled, or will enroll in the EPRS during the promotion period, or
even prior to the promotion period; and,
2.2.1.2.Have properly submitted its remittance report through EPRS at least one (1)
month during the promotion period.
2.3.Incentives at stake
2.3.1. Subject to applicable tax regulations, prizes for the promotion are as follows;..
2.3.1.1. 1st prize - PhP 50,000.00 cashj , ';. % PV^^EALTH

tcamphilliealth^^^ tcamphil health 2.3.2. A portion of the cash incentives must be given to the PhilHealth Employer's Engagement
Representative (VEER) in this manner:
2.3.2.1.for the 1st to 3rd prize winners - 10% of the incentives
2.3.2.2.for consolation prize winners - 30% of the incentives
2.4.Mechanics
2.4.1.Using a computerized raffle system, PhilHealth shall determine the winners on
January 11,2013.
2.4.2.The venue of the raffle will be at the PhilHealth Head Office, CityState Centre
Building, 709 Shaw Boulevard, Pasig City.
2.5.Guidelines on how to claim prizes
2.5.1.The luck)' winners will be informed immediately via registered mail signed by the
President & CEO of PhilHealth.
2.5.2.The prizes will be awarded during PhilHealth's celebration of its 18th Anniversary in
February 2013.
2.5.3.In cases where an employer will not be able to attend the awarding activity, the prize
will be sent to the concerned PhilHealth Regional Office, and then should be picked
up by die PhilHealth Employer Engagement Representative (PEER), or any authorized
representative of the winning employer.
2.5.4.The prizes will be in the form of a PhilHealth check, and will be drawn in the name
of the Head of Office in the case of government agencies, or in the name of the
business in the case of private business entities. PhilHealth shall draw a check in
favor of the PhilHealth Employer Engagement Representative (PEER) for his/her share of
the prize.
2.5.5.In claiming the prize, the representative of the winning employers or the PhilHealth
Employer Engagement Representative (PEER) must present any two (2) of his or her
government-issued identification, such as, PhilHealth PIN Card, Driver's License,
SSS ID, GSIS ID, or passport, in addition to his or her Company ID.
